3) Stroke Nurse Service – link between hospital and home

The Chest Heart & Stroke Scotland (CHSS) Stroke Nurse Service provides individualised information, advice and support to people affected by a recent diagnosis of Stroke / Transient Ischaemic Attack (TIA) and their carers. The stroke nurses main aim  is to support individuals to achieve their personal goals, within their capabilities. They have local knowledge of health, statutory and voluntary agencies and can coordinate and make referrals for these specialist services.

CHSS stroke liaison nurses are available (December 2015) in the following areas.

  • NHS Dumfries & Galloway
  • NHS Fife
  • NHS Grampian
  • NHS Highland
  • NHS Lanarkshire
  • NHS Lothian

Information about services in other health boards can be found by contacting your local health service

CHSS stroke nurses can also help by supporting an application to the welfare grants section of CHSS for one off equipment which cannot be sourced by local authority or NHS funding. This may include applications for short breaks or household goods if you are on a low income.
